We have made HotDocs Online templates out of the forms that need
to be completed for issuance of the death certificates.
Persons completing these forms can access them on-line and
complete them on line as well. Once the information is entered,
signature ready documents can be printed. Because we are using
HotDocs Online, there is no special software required. All the
attorney needs is a PC, internet access, a wordprocessor and a
printer.
Users can log onto lexisOne (http://www.lexisone.com.)
From the lexisOne Home Page we are adding the following link:

This will take people to the America Unites Page

From America Unites Page add the following link and text:
Procedure and Forms for Issuance of Death Certificates for
Individuals Currently Missing at the World Trade Center Access
the expedited processes and complete the fillable forms online.
This should link to a new page. As soon as I know the URL for the
page I will give it to you so you can link directly there.
Otherwise, you can link to either the lexisOne Home Page or the
America Unites Page. This is probably best because it has other
resources and information there as well.
I have asked the developers to create a new page
with copy as follows and Links to the Individual Forms Copy for
this new page is attached in the following documen. Along with
information about the procedure, we have included instructions on
how to
complete the forms online and print them out (6 easy steps). These
forms will be accessible to anyone with Internet access. Thus, Bar
members can access them at home or their offices as well. We
anticipate that this will be up and running tomorrow.
